General Notes: Husband - Dr. Leslie Ramsey Hazlett


After completing his early schooling in the district schools of Butler Township, Butler County, Pennsylvania, he attended Edinboro State Normal School, and graduated therefrom in 1890. He taught school for a number of years in Butler, Allegheny, and Westmoreland counties, and later began the study of medicine at Jefferson Medical College, Philadelphia, from which he received his degree as Doctor of Medicine in 1896. He engaged in general practice at New Galilee, in Beaver County, Pennsylvania, and after five years came to Butler. At various times beginning in 1902 Doctor Hazlett took post graduate work in eye, ear, nose and throat work in Philadelphia and New York City and after 1914 limited his practice entirely to that particular field. He was a skilled physician and surgeon and developed an extensive practice.
He was identified with the Butler County Medical Society, Pennsylvania State Medical Society, American Medical Association, and American Academy of Optholomology and Oto-Larynology. He held membership in the United Presbyterian Church, Masonic Lodge, Independent Order of Odd Fellows, Rotary Club, Butler Country Club, and Chamber of Commerce. He was a Republican.
